Hazard Health and Rehabilitation Center is a 200 bed facility nestled within the scenic mountains of Perry County in the city of Hazard, Kentucky. Since the time of our opening in March of 1976, we have been providing quality care to the residents of Eastern Kentucky and the surrounding area.

Our primary purpose is to achieve and maintain the most excellent quality of life possible for each resident. This is made possible by the tireless efforts of our dedicated medical staff, the aid of an involved community, and our many employees that give a part of their hearts and souls to the residents they serve each day. As you walk down the hallways of Hazard Health and Rehabilitation Center you can see, hear and feel the special love, respect and caring between residents and employees.

Hazard Health and Rehabilitation Center offers a variety of engaging daily activities to reflect our residents' interests. These activities are designed to help give our residents the opportunity to not only maintain, but improve their quality of life.

Our facility has an elegant and home-like atmosphere beautifully decorated to fit each season and holiday. Resident rooms are filled with personal items that reflect their past to give them a feeling of home.
